The forms attached below are for projects being funded by Evolve EB. The forms are there to help the project coordinators complete the relevant information needed to process grant claims received by Evolve EB. It is important that the correct forms are used and we do require hard copies with a signature to validate the claims. If you require more information regarding the administration of projects please contact your Community Grants Manager.

Grant claim form to claim NET

Grant claim form to claim NET amount for project. The grant claim form is for the project coordinator to request payment for work done on the project. Please ensure the relevant invoices are attached as evidence of completed work and to show it is within the guidelines of the agreed criteria set out in the original application.

Project Reporting
Form

Reporting form. The reporting form is to enable you to describe the current progress your project has made towards achieving the aims and objectives as outlined on the application.

Press Release Form

Press release form. This form is to inform Evolve EB of any publicity that is being arranged for the project. It is important that this information is sent so that any arrangements prior to the publicity can be finalised.
